Responsibilities:

• Develop and maintain effective relationships with healthcare providers, skilled nursing facilities, and rehabilitation centers to ensure smooth transitions and coordination of patient care

• Facilitate communication and collaboration among healthcare providers, facilities, and patients to ensure continuity of care

• Conduct regular assessments and evaluations of patient needs to identify and address gaps in care

• Identify and address potential issues or concerns, leveraging solutions and resources to resolve them

• Participate in quality improvement initiatives and contribute to the development of care protocols and standards

• Provide emotional support and education to patients and their families, empowering them to take an active role in their care

Requirements:

• Current RN or LPN licensure in the state of Alabama

• Strong communication and interpersonal skills, with the ability to build trust and rapport with healthcare providers, patients, and families

• Experience in liaison or coordination roles, preferably in a healthcare setting

• Strong analytical and problem-solving skills, with the ability to think critically and develop creative solutions

• Ability to work independently and prioritize tasks effectively
