Home Care Physical Therapist - Part Time Ignite Rehab, part of Metro Physical & Aquatic Therapy, provides physical and occupational therapy services. We seek a motivated part‑time Physical Therapist to deliver in‑home care to patients across Somerset County. All experience levels are encouraged to apply; new graduates will receive training. Job Details Job Type: Part‑time with benefits, up to 20 visits per week Location: Somerset County Schedule: Flexible (any 7 days of the week) Compensation: $65,000 – $70,000 per year Benefits Professional growth and development opportunities Supportive, collaborative work environment Patient‑centered care; use of Quick EMR Flexibility and autonomy: see patients in primary geographic area Responsibilities Accept and treat patients within designated geographic area Evaluate patients' needs, develop individualized treatment plans and goals in consultation with physicians, patients, and families Plan, prepare, and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques Provide timely and accurate documentation for each visit Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning Deliver care in compliance with agency policies and professional practice standards Qualifications Graduate of a Physical Therapist program Licensed Physical Therapist in the state of New York or eligible Preferred: First Aid, AED, and CPR certifications Home health experience preferred (mentorship provided for documentation and compliance) Ability to travel to patients' homes within assigned geographic territory The anticipated base salary range for this position is $65,000–$70,000, based on experience, skills, and geography.
