Job Description Loyal Source is seeking experienced Registered Nurse Case Managers for employment opportunities in a detention facility setting. DESCRIPTION OF DUTIES
The RN Case Manager is a key member of the multidisciplinary healthcare team within a detention facility. This role focuses on coordinating and managing the healthcare needs of individuals in custody by integrating clinical assessment, chronic disease management, care coordination and transfer planning.
Working within a 24/7 primary care environment (medical, dental, behavioral health, and administrative staff), the RN Case Manager ensures timely access to appropriate services, continuity of care within the facility, and safe transitions of care to other correctional facilities or community providers upon transfer or release. The RN Case Manager also supports quality improvement initiatives, policy implementation, and compliance with regulatory and contractual requirements. Pay Rate: 35.89/hr
1. Clinical Case Management
- Conducts comprehensive nursing assessments, intake screenings, and health histories to identify medical, mental health, and dental needs requiring ongoing case management.
- Develops, implements, and updates individualized care plans for patients with chronic illnesses, complex medical conditions, or high-risk status.
- Coordinates ongoing care for patients with serious or chronic conditions (e.g., diabetes, hypertension, cardiac disease, infectious diseases, mental health disorders, pregnancy).
- Monitors patient progress and outcomes, adjusting care plans in collaboration with medical, dental, and behavioral health providers.
- Facilitates continuity of care for patients in special housing units, segregation, and infirmary settings when applicable. 2. Care Coordination & Transitions of Care
- Coordinates referrals and follow-up with onsite and offsite providers, including specialists, hospitals, and behavioral health services.
- Collaborates with detention staff, case workers, and external agencies to plan safe and effective transitions of care (e.g., transfers between facilities, releases to the community).
- Prepares and organizes medical records, summaries, and discharge instructions for patients being transferred or released.
- Ensures appropriate follow-up appointments, medications, and patient education are arranged at the time of discharge, when feasible and appropriate. 3. Utilization Management & Access to Care
- Reviews requests for offsite specialty care, diagnostics, and procedures, ensuring they are clinically appropriate and timely.
- Tracks and follows up on pending consultations, testing, and procedure results, escalating concerns when necessary.
- Identifies and addresses barriers to access, working with the healthcare team and facility leadership to improve processes. 4. Direct Nursing Care
- Provides direct patient care within the RN scope of practice, consistent with current accepted standards, licensure, certification, and competency as needed.
- Performs sick call assessments and treats acute problems using nursing protocols, referring to providers as appropriate.
- Conducts physical examinations and intake screenings using established tools, referring abnormal findings to medical, dental, or behavioral health providers.
- Administers medications via approved routes as ordered.
- Performs treatments such as wound care, vital sign monitoring, electrocardiograms, and point-of-care testing.
- Interprets specimen results or routes results appropriately, and initiates follow-up actions per protocol.
- Responds to medical emergencies, stabilizes patients, and coordinates transfer to appropriate levels of care. 5. Coordination with Multidisciplinary Team
- Collaborates closely with physicians, advanced practice providers, dentists, behavioral health providers, and other team members.
- Provides clinical supervision to Licensed Vocational/Practical Nurses and unlicensed assistive personnel, as assigned.
- Participates in case conferences, interdisciplinary rounds, and care planning meetings.
- Serves as a resource and advocate for patients, ensuring their healthcare needs are identified, communicated, and addressed. 6. Documentation, Compliance & Quality Improvement
- Documents all assessments, interventions, referrals, case management activities, and patient education in the electronic health record (EHR) accurately and timely.
- Ensures compliance with facility policies, applicable standards (e.g., NCCHC, NDS), and regulatory requirements (including HIPAA/Privacy Act).
- Participates in quality improvement, performance measurement, and risk management initiatives.
- Assists with audits, data collection, reporting, and implementation of corrective actions as needed.
- Supports infection prevention and control practices throughout the facility.
REQUIRED EDUCATION AND EXPERIENCE
1. Degree-issuing institution and/or program must be accredited by an entity recognized by the U.S. Department of Education.
2. Graduate from an associate, baccalaureate, or graduate nursing program for Registered Nurses located in a State, the District of Columbia, or a U.S. territory and accredited by:
- The Accreditation Commission for Education in Nursing (ACEN), or
- The Commission on Collegiate Nursing Education (CCNE).
In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
3. Minimum of one (1) year of professional experience as a licensed Registered Nurse in a hospital, medical office, correctional, detention, or residential setting.
4. Prior experience in case management, care coordination, discharge planning, or chronic disease management is preferred.
5. Experience in a detention/correctional healthcare setting is preferred. REQUIRED LICENSURE / CERTIFICATION
1. Current, full, and unrestricted license as a Registered Nurse in the state in which the duty station is located, or a compact state that allows practice in the duty state. Any changes to license status must be reported to the employer immediately.
2. This position does not require privileging.
3. Maintains current BLS for Healthcare Providers® certification through the American Heart Association®.
4. Certified Correctional Health Professional (CCHP), Certified Correctional Health Professional - Registered Nurse, Certified Corrections Nurse (CCN), or an accredited Case Management certification (e.g., CCM, ACM) is preferred but not required. ATTRIBUTES AND PROFESSIONAL QUALITIES

1. The facility operates 24/7/365; this position may be required to respond during an emergency activation.
