Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our evening shift team. As a LPN at Autumn Lake Healthcare at Long View, you will have the opportunity to make a meaningful difference in the lives of our residents. Job Responsibilities:
Provide direct patient care to our residents, including medication administration, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Maintain accurate and up-to-date records of patient care and treatment
Report any changes or concerns to our management team
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Qualifications & Experience Requirements:
Previous experience as a Licensed Practical Nurse (LPN) is preferred
Must hold a current Licensed Practical Nurse (LPN) License
Schedule:
Evenings (2:30 pm to 11:00 pm)
