We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.Physical Therapist (PT) Details:Location: Dayton, OH & Surround AreasSchedule: PRN | Monday - Friday; Flexible SchedulingPay per Visit: $60.00 - $120.00 (Based on Visit Type)Work Setting: On the Road for Patient Visits & Training at the Local Office or RemotelyTerritory: ~30-Mile Radius (Mileage Reimbursed)What You'll Do:As a PT, you’ll play a vital role in helping patients regain mobility and independenceDelivering physical therapy services per physician’s care plan and state regulationsAssessing patient mobility, strength, and rehabilitation potentialDesigning and implementing personalized therapy programsEducating patients and caregivers on therapeutic techniques and equipmentCollaborating with interdisciplinary teams to ensure holistic careSupervising PTAs and home health aides as neededDocumenting progress and participating in discharge planningWhat We're Looking ForRequired:Active Ohio PT licenseDegree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)CPR certificationValid driver’s license, reliable transportation, and auto insuranceRecent TB testPreferred:2+ years of PT experienceHome health or community-based care experienceStrong communication and organizational skillsAbility to build rapport and lead with empathyWhy Summit?We offer a well-rounded benefits package to support you both at work and at home.
