Job Description

Deliver meaningful care and impactful patient education as a Nurse Practitioner conducting home health risk assessments on Medicare Advantage members in Shelby County, OH. This per diem opportunity allows you to serve patients in their homes, supporting overall wellness and working to close gaps in care without the demands of ongoing treatment planning or prescribing.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Set your preferred minimum weekly or bi-weekly hours

Key Responsibilities:

• Perform comprehensive annual risk assessments in home settings, each averaging about 45 minutes
• Review prior and current medical history, conditions, and medication regimens
• Educate patients on their health conditions and medication usage
• Identify care gaps and recommend follow-up actions for comprehensive patient support
• Complete assessments in various locations across Shelby County, including Sidney, Fort Loramie, and Anna
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited school of nursing
• Minimum of 6 months current NP experience preferred
