Mileage Reimbursed. Flexible Scheduling. Responsibilities Evaluate, administer, and oversee evidence‑based physical therapy services for patients with physical disabilities or functional limitations. Develop and progress a plan of care to meet patient needs and assist them in returning to prior level of function, in accordance with physician referral guidelines, state and national standards, and departmental policies. Communicate and document with attending physicians, other disciplines (RNs, CNAs, OTs, STs, MSWs), and case managers regarding patient condition, discharge planning, plan of care, patient/family problems, and third‑party payer issues. Provide home care support for patients and families via skilled visits and joint home visits with the care team. Ensure patient and family needs are met through the care plan in collaboration with attending physicians and other disciplines. Develop, implement, and oversee a “therapy only” care plan and collaborate with the health‑care team to produce a cohesive, comprehensive plan of care. Manage an independent caseload of patients and monitor provision of care for appropriate outcomes, visit frequency, and documentation compliance. Implement individualized treatment plans using therapeutic exercises, manual techniques, functional mobility training, and modalities as indicated. Provide verbal and written instructions to patients, caregivers, and family members as needed. Assist with essential department management functions, including participation in business management activities, leading process improvement, evaluating compliance to standard work, and facilitating ongoing quality assurance and improvement programs. Education Bachelor’s in Physical Therapy from an accredited university approved by the APTA. Doctorate in Physical Therapy from an accredited university approved by the APTA. Master’s in Physical Therapy from an accredited university approved by the APTA (preferred). Experience 1 year in related experience (required). 1 year in a leadership position (required). 2 years in Home Health experience (required). Licenses & Certifications Physical Therapist license, Arizona State Board of Physical Therapy (required). Basic Life Support (BLS) certification (required). Driver’s License (required).
