Community Re-entry Planner

Pay Range Anticipated Hiring Range: $25.25 - $28.88 hourly. Full Pay Range: $25.25 - $39.75 hourly. Position eligible for overtime compensation for hours worked over 40 in the designated workweek. Posting Date 06/24/26 Application Deadline 07/08/26 Job Type Unclassified Department Correctional Health About The Position Community Re-entry Planner. Responsible for guiding justice‑involved individuals transitioning from Maricopa County jails back into the community. Supports the CHS Community Transitions Division and works closely with the Opioid Treatment Program (OTP) to screen patients, provide OUD medication education, connect community clinics, and submit referrals. Responsibilities Screen patients for the OTP and provide education on medications that treat Opioid Use Disorder (OUD). Facilitate connections to community clinics for ongoing support after release and submit referrals. Coordinate with a multidisciplinary team to develop patient discharge plans, including community entities. Assist qualifying patients in AHCCCS enrollment. Obtain treatment records and coordinate care with family and community providers relevant to the patient’s care and re‑entry plan. Maintain and update a list of community resources to support patients’ transition back to the community. Job Contributions Facilitate discharge planning with outside providers, vendors, and stakeholders for inmates at risk due to medical, mental health, or substance use issues. Develop and coordinate community re‑entry plans addressing risk factors associated with recidivism. Ensure warm handoff from jail to assigned provider or Mercy Care Arizona provider upon release. Maintain HIPAA patient confidentiality and comply with county policies on protected health information. Model the code of ethics of licensure and meet regulatory, licensure, accreditation, health assessment, and CPR requirements. Qualifications One year of full‑time behavioral health work and/or substance use experience. Bachelor's degree in Behavioral Health, Social Services, or a related field. Other Requirements Complete Health‑e‑Arizona Plus (HEA+) training within the first three months of hire to become a certified correctional assistor for AHCCCS applications. Possess or have the ability to obtain a valid Arizona driver’s license by the time of hire. We Also Value Demonstrated case management and discharge planning experience. Experience working with RBHA services and criminal justice processes. Intermediate proficiency in building Excel reports to coordinate and track discharge status. Experience working with an electronic health record. Bilingual skills in English and Spanish. Working Conditions Travel or float to various clinical locations within a 5-mile radius. Respond to emergencies and perform CPR / chest compressions as required. Work inside a jail and stand/walk/bend for extended periods of time throughout the shift. Pass a fingerprint background check and maintain MCSO jail access. Handle a high volume of work, communicate with a culturally diverse patient population, and adapt to frequent changes, interruptions, and unscheduled tasks.
