Position Overview

We are seeking a dedicated Registered Nurse Case Manager to provide skilled, patient‑focused home health care within Luzerne County. This position is responsible for managing a defined territory and delivering comprehensive nursing services across the home health continuum. The role is 100 percent patient care and requires collaboration with team members to ensure continuity of services when covering for scheduled time off.

Schedule & Work Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 32 hours
• Productivity Expectation: 30 points per week
• Coverage Area: Luzerne County
• On‑Call/Weekend Requirement: Every third weekend and one night of on‑call coverage per week
• Mileage Reimbursement: Yes, reimbursed between patient visits
• Pay Rate: $51 to $53 per hour

Key Responsibilities

• Serve as RN Case Manager for an assigned territory, providing direct patient care and maintaining accountability for assigned patients
• Perform skilled nursing visits including routine visits, start of care, resumption of care, recertifications, and discharges
• Complete comprehensive patient assessments and develop, implement, and update individualized plans of care
• Coordinate care with physicians, interdisciplinary team members, patients, and families to support optimal outcomes
• Provide patient and caregiver education related to disease processes, medications, and safety
• Maintain accurate, timely documentation in the electronic medical record
• Support team coverage by assisting with visits when colleagues are off to ensure continuity of care
• Adhere to agency policies, infection control standards, and state, federal, and CMS regulations

Required Qualifications

• Active Registered Nurse license
• Ability to work independently in a field‑based role
• Strong clinical assessment, organization, and documentation skills

Preferred Qualifications

• Recent home health nursing experience
• Experience using Homecare HomeBase (HCHB)
• Strong time management skills and adaptability
