We work with seniors and adolescent clients in their homes to assist with the following.

RN Summary

• The Registered Nurse (RN) provides skilled nursing care to clients in their homes in accordance with the plan of care, physician orders, and agency policies.

• The RN performs comprehensive assessments, delivers evidence‑based interventions, documents accurately, and collaborates with the interdisciplinary team to ensure safe, high‑quality care.

LPN Summary

• The Licensed Practical Nurse (LPN) provides direct nursing care to clients in their homes under the supervision of an RN and in accordance with the plan of care.

• The LPN supports clients with skilled tasks, monitoring, and education to promote safety, independence, and improved health outcomes.

Certified Nursing Assistant (CNA) Summary

• Perform personal care tasks aligned with SC CNA scope of practice.

• Take and record vital signs , weight, and intake/output.

• Assist with range‑of‑motion exercises and mobility programs.

• Support wound care and skin integrity monitoring (non‑skilled tasks).

Home Health Aide (HHA) Summary

• Provide personal care and homemaking services under RN supervision.

• Assist with safe transfers, bathing, grooming, and toileting.

• Observe and report changes in client condition.

Personal Care Aide (PCA) Summary

• Assist with ADLs, companionship, and home support tasks.

• Provide safety monitoring, meal prep, and light housekeeping.

• Reinforce care instructions provided by the RN.

Certified Medication Aide (CMA) Summary

• Administer medications per SC regulations and agency policy.

• Document medication administration accurately.

• Report medication refusals, side effects, or concerns promptly.

Qualifications

• Active South Carolina RN/LPN license in good standing.

• Minimum 1 year of clinical experience (home health preferred but not required).

• (LPN) Minimum 6 months of clinical experience (home health preferred).

• CNA: Active South Carolina CNA certification.

• HHA/PCA: Experience providing personal care (formal or informal).

• CMA: Active SC CMA certification.

• Valid driver’s license, reliable transportation, and auto insurance.

• Ability to lift, transfer, and assist clients safely.

• Strong communication, professionalism, and reliability.

• CPR/BLS certification preferred.
