Registered Nurse (RN) - Long Term Acute Care

We are seeking a skilled and dedicated Registered Nurse (RN) specialized in Long Term Acute Care (LTAC) to join our team. This role involves providing high-quality patient care in a supportive environment, contributing to the recovery and overall health of our patients.

Shift Details

• Shift: Night 4x12-Hour (19:00 - 07:00)
• Weekend Requirements: Yes, every other weekend
• Floating Requirements: Yes, may need to float within scope to meet facility needs
• Orientation Hours: 24 hours

Responsibilities

• Provide comprehensive nursing care to patients in a long-term acute setting, focusing on critical conditions and recovery.
• Manage a patient ratio of up to 5 patients per shift.
• Perform advanced nursing procedures including care of ventilated patients, arterial line management, and blood product administration.
• Collaborate with an interdisciplinary team, including physical therapy and respiratory services, to enhance patient care.
• Maintain accurate patient records and chart using the Epic system, ensuring compliance with facility protocols.
• Administer medications and monitor vital signs, responding to changes in patient condition promptly.

Qualifications

• Minimum of 2 years of applicable nursing experience in medical-surgical or telemetry settings.
• Preferred experience in Long Term Acute Care (LTAC).
• Required certifications: ACLS, BLS.
• Experience with the Epic charting system is mandatory.
• Demonstrated skills in managing complex care and critical patient conditions.
• No background or license actions within the past 12 months.

Patient Care Details

• Adults of various age groups with conditions such as COPD, renal failure, sepsis, and wound management.
• Daily census of approximately 20 patients with an interprofessional approach to care.
• Support for patients undergoing procedures such as hemodialysis and central line management.
