Job Details Location: Cortez, CO 81321 Position Type: Part Time Education Level: 4 Year Degree Salary Range: $31.01 - $35.37 per hour Travel Percentage: Up to 50% Job Shift: Day Job Category: Health Care Title: Diabetic Educator – Part Time – 20 Hours – Cortez or Pagosa Springs, CO Position Summary The Diabetic Educator provides nutrition referral consultations and diabetes education referral consultations to Axis Health System patients. Under the scope of the Registered Dietitian licensure and ADA certification, the educator collaborates with the integrated health team to deliver health, nutrition, and diabetes education to pediatric and adult patients, enabling self‑management and overall health. Responsibilities include individual appointments, joint visits with providers, group sessions, classes, outreach activities, coordination of ADA Diabetes Education certification program requirements, and mentorship of other Diabetes Education staff. The position is primarily based at Durango Integrated Healthcare, with periodic/monthly visits to other Southwest regional clinics and telehealth patient appointments. Qualifications & Experience Bachelor’s or Master’s degree in Nutrition or as a Registered Dietitian (RD) Diabetes CDCES or CDE certification training or actively obtaining hours to take the certification exam Registered Dietitian license At least 1 year of experience as an RD Minimum 1 year providing diabetes education under a supervised ADA program Experience coordinating ADA Diabetes Education certification program requirements Salary Starting at $38.00 - $40.28 per hour (non‑exempt, hourly), considering experience and location.
