The pay for this position is $44,850-$58,908.9150 annually.

$500.00 sign on bonus

The Skills-Based Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals. Our ideal candidate will have strong collaboration, problem-solving, and interpersonal skills

• An Associate's Degree and two years experience and/or Bachelor's Degree.
• Knowledge on how to provide holistic and therapeutic case management services.
• Experience working in the mental health field.
• Computer and documentation skills
• Ability to solve problems and deal with complex situations and stay calm
