Job Description At Elara Caring, we provide high‑quality home‑care services to more than 60,000 patients daily. We are looking for a Licensed Vocational/Practical Nurse to help deliver care in patients’ homes and support them on their health journey. Responsibilities Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines. Incorporate patient care goals established in the Plan of Care into nursing visits, as documented in skilled nursing notes. Communicate significant findings, problems, and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician as required. Implement the plan for patient safety using the patient, family, and community resources. Assist patients in learning appropriate self‑care techniques while delivering quality patient care. Suggest revisions to the plan of care in response to identified patient care issues and notify the RN Case Manager. Provide services that require substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Document medication regimen and update medication profile according to established procedures. Treat patients and caregivers with the highest and most effective level of care. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel. Qualifications Graduate of an accredited Certificate, Diploma, or Associate School of Nursing. Current State License as a Licensed Vocational/Practical Nurse. 1 year of experience in a clinical care setting. Experience in a hospice or home health environment is preferred. 50% travel required. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Valid driver’s license and insurance and reliable transportation to perform job tasks. Benefits Competitive compensation package.
