Responsibilities Deliver targeted occupational therapy interventions for residents under the supervision of aLicensed OT in a skilled nursing setting. Implement, monitor, and adjust treatment plans to progress functional goals; document progress notes and outcomes per facility policies. Conduct functional assessments, track progress, and contribute to discharge planning and home exercise program development. Collaborate with an interprofessional team (OT, nursing, rehab, social work) to coordinate care and optimize resident outcomes. Educate residents and families on activity adaptation, safety, and home programs to promote independence in daily living activities. Ensure compliance with safety, infection control, privacy, and facility policies; participate in ongoing quality improvement activities.

Requirements Active Certified Occupational Therapy Assistant license in New Mexico or eligible for licensure by endorsement Ability to work effectively in a skilled nursing facility setting with adult residents Strong communication and teamwork skills to collaborate with the interdisciplinary care team Ability to complete accurate documentation and progress notes in a timely manner Willingness to participate in a travel assignment and comply with agency and facility policies
