Licensed Practical Nurse (LPN) - Full-Time

Inspired Home Care is a rapidly growing home health agency seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our clinical team. Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes under the supervision of a Registered Nurse. This role focuses on supporting patient recovery, monitoring health conditions, and assisting with treatment plans to promote optimal health outcomes.

Key ResponsibilitiesPatient Care

• Provide direct nursing care to patients in their homes in accordance with physician orders and the established plan of care

• Administer medications and treatments as prescribed

• Perform basic nursing procedures including wound care, vital sign monitoring, and patient assessments

• Observe and report any significant changes in patient condition to the supervising Registered Nurse

Care Coordination

• Collaborate with physicians, registered nurses, therapists, and other members of the interdisciplinary healthcare team

• Assist in implementing individualized patient care plans

Patient and Family Education

• Educate patients and caregivers regarding medication administration, treatment plans, and home safety practices

• Reinforce patient education provided by the Registered Nurse or other healthcare professionals

Documentation and Compliance

• Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements

• Ensure compliance with HIPAA regulations and home health care standards

Travel

• Travel to patient homes within assigned territories throughout Maricopa County

Qualifications

• Current Arizona Licensed Practical Nurse (LPN) License in good standing

• Previous home health or clinical experience preferred

• Strong clinical, organizational, and communication skills

• Ability to work independently in a field-based healthcare environment

• Valid driver's license and reliable transportation
