Role Overview The RN Case Manager (Inpatient Care Coordinator - ICC) plays a crucial role within the interdisciplinary team, overseeing patient care from admission to transfer or discharge. Their primary focus is optimizing resource utilization while ensuring effective discharge planning and patient advocacy. Department: Case Management Shift: Monday to Friday, 08:00 – 16:30 (40 hours per week) Rotation: Includes rotating weekends Experience Required: Minimum 2 years of recent acute care hospital discharge planning experience Key Responsibilities Care Coordination : Direct and coordinate patient care throughout their hospital stay, ensuring seamless transitions.
