Job Opportunity

Position: Occupational Therapist - Home Health

Location: Sacramento, California

Role Responsibilities:

• Prioritize start of care visits for patients.
• Supervise Occupational Therapy Assistants in the field.
• Complete mandatory reporting of start of care visits the next day.
• Submit clinical notes and other documentation within 48 hours following each visit.
• Maintain timely communication with supervisors, intake teams, and office staff.
• Be generally responsive during standard business hours.

Requirements:

• Valid California Occupational Therapy License.
• Minimum one year of experience in Home Health.

Territory:

Alameda County, with potential expansion as needed.

Schedule:

40 hours per week, Monday to Friday.

Training:

Three full days of training: one in-house and two in the field.

Compensation Details:

• Mileage reimbursement at $0.655 per mile (from first to last patient).
• Parking and toll expenses are covered.
• Company car option with a reduced bill rate available.

Productivity Requirement:

30 points per week.

Point to Hourly Conversion:

1 point = 1.33 hours. Caseloads and visit types are structured to ensure an average of 8 hours per day and 40 hours per week. Overtime requires prior approval from the Clinical Supervisor.

Visit Types and Point Values:

• Start of Care: 2 points
• Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
• Revisit, Non-OASIS Discharge: 1 point
