The RN will manage care plans, administer treatments, and collaborate with multidisciplinary teams to support patients in their homes.QualificationsActive Connecticut RN licenseMinimum 2 years of nursing experience (home health preferred)Reliable transportation for patient visitsBLS Certification preferredStrong communication, organizational, and critical thinking skillsKnowledge of OASISWhat You’ll DoPerform clinical assessments and provide nursing interventions including wound care, medication administration, and proceduresMonitor and document changes in patient conditionExecute and follow up on physician ordersCollaborate with home health aides and other care providersManage care coordination and ensure quality, patient-centered careCompensationHourly: $65.00 - $80.00 per hour
