As an Occupational Therapist, you will play a crucial role in evaluating and implementing rehabilitation interventions, managing patient care, and educating patients and their families. Your expertise will help improve patients' quality of life by promoting independence and functional abilities.
- Evaluate and establish individualized care plans to meet patients' needs in the home environment
- Coordinate care with other disciplines and seek orders for additional services as needed
- Provide direct patient care based on established care plans and physician orders
- Apply evidence-based practice guidelines and Medicare regulations in evaluations and treatment techniques
- Modify treatment plans as appropriate, including frequency and duration of care
- Master scoring of CMS-required OASIS and collaborate with the Quality team for adjustments
- Participate in multidisciplinary conferences to establish relevant care plans
- Document all skilled services, adverse events, and communications in the medical record
- Engage in Quality Assurance Performance Improvement initiatives
- Manage productivity standards and maintain accurate time logs and mileage
- Oversee services provided by Occupational Therapy Assistants and supervise CNAs when needed
- Maintain occupational therapy licensure and participate in educational opportunities
