Practice Overview:

* Dedicated hospice and palliative care teams serving patients for 40 years.

* Inpatient Hospice Unit at main campus- 7 bed unit

* Free standing 16 bed hospice currently under construction just minutes from main campus

* Provide support to nursing and interdisciplinary staff by providing oversight for home hospice and in-home palliative care

* Full-time and part-time schedule available

* Service Line supported by dedicated:

o Physicians and Advanced Practitioners

o Licensed Clinical Social Workers

o Chaplains

Working together with Nurse Practitioners like you:

* Board Certified FNP or AGACNP

* Active Georgia license or ability to obtain unrestricted license

* Passion for quality and patient care

* Experience in Palliative or Hospice Care preferred

* Functions well in a team environment, contributing to and supporting organizational goals and initiatives
