Community Health Worker

Position Overview

WiredPeople, Inc. is seeking a dedicated and compassionate Community Health Worker (CHW) to join our Community Engagement Team. In this role, you will conduct direct community outreach to help individuals and populations at risk of health disparities navigate the healthcare system.

Working closely with community partners, you will deliver culturally appropriate health education, social support, and informal counseling while advocating for the health and basic needs of local community members. This position is full-time up to 37.5 hours a week and will require some travel to Dillion County. 

Note: Required CHW certification and role-specific program training will be provided upon hire.

Key Responsibilities
1. Community Outreach & Engagement
  • Conduct individual-focused outreach directly within local communities to identify and understand public health needs.

  • Engage community members and provide resources and technical assistance to achieve long-term, sustainable, and inclusive health outcomes.

  • Promote equitable health decision-making processes across diverse populations.

2. Care Navigation & Client Support
  • Assist clients in navigating complex healthcare and social service systems.

  • Serve as a vital liaison between community members and available healthcare providers and support agencies.

  • Issue appropriate referrals and distribute approved educational resources to help clients increase health literacy, self-sufficiency, and self-care management.

3. Community Collaboration & Partnership
  • Collaborate with local organizations and community partners to support outreach initiatives and referral networks.

  • Build active community involvement and establish peer-to-peer relationships to connect residents with essential resources.

4. Community Advocacy
  • Advocate for the needs, perspectives, and well-being of community members.

  • Assist clients in accessing fundamental resources to ensure their basic health and social needs are met.

5. Data Management & Reporting
  • Conduct individual needs assessments as requested or required.

  • Document social and health encounters to assist evaluation teams in identifying health trends.

  • Track and monitor referral data, client demographics, and resource utilization.

  • Prepare clear, concise data reports for agency leadership and program grantors.

6. Additional Duties
  • Perform other related duties and support emergency response plans as assigned.

Qualifications & Requirements
Minimum Requirements
  • High School Diploma or GED.

  • At least one (1) year of experience working in a community-based setting (includes paid or volunteer experience with local community organizations).

  • Valid driver’s license or reliable personal transportation to travel across local community sites.

Preferred Qualifications
  • South Carolina Certified Community Health Worker (SC CCHW) credential (preferred upon entry; training provided if not currently certified).

Knowledge, Skills, and Abilities
  • Empathy & Advocacy: Strong ability to empathize with public health patients and advocate effectively for their needs.

  • Communication: Excellent oral and written communication skills to engage with diverse community members, partner organizations, and internal teams.

  • Confidentiality & Ethics: Ability to maintain strict patient confidentiality and handle sensitive information professionally.

  • Independence & Adaptability: Self-driven with strong organizational skills; able to follow instructions, multi-task, and work independently with direct supervision.

  • Flexibility: Willingness to be cross-trained across various public health initiatives and adapt to evolving community needs.

Working Conditions & Physical Requirements
  • Work Environment: Mix of office environment and active field/community settings.

  • Travel: Regular travel within local communities is required.

  • Schedule: Standard schedule is 37.5 hours per week. Occasional evening work (after 5:00 PM), early morning hours (before 8:30 AM), or weekend hours (Saturdays) may be required.

  • Physical Demands: Ability to lift up to 30 pounds and stand for extended periods during community outreach events.

  • Emergency Response: Position participates in DPH Emergency Plans and may be subject to 24-hour call during public health emergencies or disasters.
