Job Description & Requirements

Required Qualifications

• Graduate of an accredited Associate Degree in Nursing (ADN) program or Nursing Diploma program.
• Current Registered Nurse (RN) license in the applicable state.
• Current CPR certification.
• Valid driver's license.
• Reliable transportation and proof of insurance.
• 1–2 years of experience as a Registered Nurse in a clinical care setting.
• Minimum 2 years of home health nursing experience.
• OASIS experience required.
• Homecare Homebase (HCHB) software experience required.

Required Skills & Experience

• Strong assessment and critical-thinking skills.
• Knowledge of medication administration, IV therapy, wound care, and home-based patient care.
• Ability to manage multiple patients independently while prioritizing care needs.
• Excellent communication and collaboration skills.
• Ability to work independently and effectively in a home care environment.
• Commitment to providing safe, compassionate, patient-centered care.
• Ability to travel locally and maintain a flexible schedule.
