Care Management Assistant, PT, Days

Overview:
Working hours are Saturday and Sunday 8:30- 5 or 8-4:30pm with potential shifts open on weekdays if available. Training MUST be Monday - Friday for the first few weeks
Responsibilities: As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments. Qualifications: Education- H.S. Diploma or General Education Degree (GED) Required
- Associate’s Degree or two years of college in human services/health related discipline Preferred
- 2 years of experience in a healthcare or social services setting Required
- None Required
