Summary:

Wound Care Nurse:

The Wound Care Nurse is responsible for assessing, planning, implementing, and evaluating wound care processes in the facility.

Essential Duties and responsibilities:

1. Develops process improvement plans for identified gaps to facilitate a reduction in facility acquired wound infections and pressure ulcers.

2. Provides direct patient care for all types of wounds including but not limited to traumatic injuries, surgical sites, ostomies, pressure ulcers and incontinence related skin problems. Works with patients and families to provide education and training on the care of wounds.

3. Educates all members of the healthcare team on ordered treatments.

4. Maintains documentation on all residents involved in Wound Care treatment

5. Collaborates with physicians, nursing, and CNA's to provide optimal impatient care including recommendations for post discharge wound care needs.

6. Is familiar with the care and use of a Wound VAC, the ordering of supplies and the application or discontinuation of the treatment.

7. Educates all members of the healthcare team on the care and usage of the Wound VAC.

8. Prepares a weekly report of all Decubitus ulcers, moisture splits, significant skin breakdown, pressure ulcers and all significant skin problems.

9. Maintains an adequate inventory of all potential wound care needs and supplies.

10. Attends infection control meetings, all in-services as required, and all mandatory meetings.

11. Maintains contact with the D.O.N. on all residents that require treatment.

LICENSE/CERTIFICATION REQUIREMENTS :

Minimum: Current MI Nursing License, Wound Care Certification. Current BLS certificate

EXPERIENCE REQUIREMENTS :

Minimum: 5 years nursing experience, 1 year wound care experience

KNOWLEDGE/SKILLS :


Excellent verbal and written communication skills. Ability to use critical thinking skills for assessment and planning. General computer skills including Microsoft Word, Experience with electronic documentation and EMAR administration of medications.

Working Conditions:

Exposed to unpleasant sights, sounds, and smells.

Must be able to lift 75 pounds.

May be exposed to violent, confused residents who are uncooperative.
