Mental Health Registered Nurse Case ManagerGiaCare is currently seeking a full-time Mental Health Registered Nurse Case Manager to support the 49th Medical Group at Holloman Air Force Base in Alamogordo, New Mexico.This position offers experienced Registered Nurses the opportunity to provide comprehensive case-management services to members of the military community in a collaborative outpatient environment. The RN Case Manager will coordinate care across inpatient, outpatient, community, and telephonic settings while supporting continuity of care and optimal patient outcomes.Schedule:Full-time, Monday through FridayEight working hours scheduled between 7:00 AM and 5:00 PMOne-hour unpaid lunchNo weekendsNo on-call requirementsFederal holidays offSchedule provided approximately one month in advanceBenefits:Paid time offPaid federal holidaysMedical, dental, and vision insurance options401(k) retirement planResponsibilitiesAssess patient needs and develop, implement, coordinate, and monitor individualized care plansProvide case-management services through inpatient, outpatient, onsite, community, and telephonic settingsCoordinate treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventionsSupport continuity of care and follow-up following dischargeIntegrate nursing case management with utilization management and social-work case managementCollaborate with physicians, nurses, social workers, caregivers, community organizations, and other members of the healthcare teamIdentify appropriate healthcare and community resources for patientsMaintain accurate patient-care records and case-management documentationProvide health education and instructions to patients and caregivers based on identified needsAssist in developing and implementing patient-assessment tools, clinical guidelines, algorithms, case-management processes, and community-resource databasesParticipate in quality-improvement activities, meetings, training, and other approved administrative functionsMinimum QualificationsU.S. Citizen, U.S. national, lawful permanent resident, or individual otherwise authorized to work in the United StatesAssociate's Degree of NursingCurrent, full, active, unrestricted Registered Nurse license in any U.S. state, territory, or the District of ColumbiaAt least one year of professional nursing experience following graduationCurrent Basic Life Support (BLS) certification (or willing to obtain)Demonstrated clinical competency within the previous two yearsCase Management Certification:Candidates who do not currently possess an approved case-management certification may still qualify but must obtain one within the first 12 months of employment.Approved certifications include:Certified Case ManagerCertified Disability Management SpecialistCertified Rehabilitation Registered NurseCertified Occupational Health NurseCertified Occupational Health Nurse-SpecialistAdvanced Certification in Continuity of CareCertified Rehabilitation CounselorRN Nurse Case ManagerCare Manager CertifiedWhy GiaCare?GiaCare is dedicated to supporting our nation's military by connecting exceptional healthcare professionals with military treatment facilities throughout the United States.
