PRIMARY CARE PHYSICIAN

Location: Montgomery, AL

Compensation: $250,000 - $295,000

Position Summary

Seeking a Primary Care Physician to provide high-quality patient care in a collaborative, value-based care environment. Physicians are supported by a strong interdisciplinary team including advanced practice providers, pharmacists, care coordinators, behavioral health staff, medical assistants, and referral coordinators allowing more time with patients and lower daily patient volume.

Key Responsibilities

• Evaluate and treat patients according to clinical standards
• Coordinate specialty referrals, hospital/SNF care, home health, and DME services
• Participate in daily care team huddles
• Maintain timely and accurate EMR documentation
• Collaborate with leadership on quality and patient outcomes
• Provide patient-focused care in a team-based setting

Qualifications

• MD or DO from an accredited program
• Active or eligible medical license
• Board Certified/Board Eligible in Family Medicine, Internal Medicine, or Geriatrics preferred
• Active DEA license
• Strong communication and patient care skills
• Comfortable working in a value-based care model
• EMR/basic computer knowledge required

Preferred

• Knowledge of HEDIS/STARS, CAHPS, and HOS
• Experience with Medicare Advantage or underserved communities
