Job Title : Ambulatory Nurse Clinician- Neurology

Work Schedule: The is a full-time hybrid position with on-site and remote work. The schedule flexes based on provider clinic schedule.

Location:

Up to 3 days of clinic which could be at main campus in MKE or one of closer to home sites, New Berlin, Delafield, Mequon, or Forest Home depending on where the provider does. Minimum of 2 days a week at home. However, this could change based on coverage needs and needs to be flexible.

What you will do:

Triage Phone Response: Manages patient health concerns and symptoms via a telephone interaction. These telephone interactions require assessment, patient education, and critical thinking. Responds to and prioritizes incoming patient messages and test results. Assisting with refills per protocol. Delivers patient education, according to the individualized needs of the patient Educates patient and/or family members about the diagnosis, diagnostic procedures, and medications,

Clinic Support: Collaborative relationships support the providers during clinic by creating the AVS, scheduling appointments, and education of patient/family.

Care Coordination: Strengths in critical thinking and prioritization to coordinate the patient care outside of the visit. Answering family phone calls and My Chart messages. Refilling medications. Education to patient/family. Collaborative work with insurance/pharmacy to ensure minimal disruptions of medication dispensing. Ability to adjust communication to meet the needs the diversity of our population.

The Nurse Clinician promotes optimal wellness by promoting health maintenance, participating in the management of acute illness, managing the effects of chronic disease and disability across the continuum of care. This is accomplished through face to face or electronic interactions. Utilizing critical thinking and nursing process in caring for patients and caregivers with complex healthcare needs which are acquired through a process of integrating education, experiential knowledge and evidence-based guidelines.
Acting as a partner, advocate, support and teacher to the patient and caregivers providing optimal management of the child's health care, and respecting their culture, values, diverse individual needs, health goals and treatment preferences. The Nurse Clinician facilitates continuity of care using the nursing process, inter-professional collaboration and care coordination activities to support the whole child by assessing and providing access to appropriate health care services and community resources across the care continuum.

What you will need:

• Current license to practice nursing in Wisconsin.
• Current CPR certification - Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required.
• Associates degree in Nursing
• Minimum of 2 years of Pediatric Nursing experience with the emphasis on managing patients with chronic and complex family and health needs.
