Mental Health Association of the Southern Tier Base Pay $32.00 - $35.00 / Hour Job Category Crisis Stabilization Center, Registered Nurse Employee Type Non-Exempt FT Manage Others No Contact information Name: Olivia Shimkus Phone: View phone number on click.appcast.io Job Summary The RN Care Coordinator for the Supportive Crisis Stabilization Center provides basic and essential medical care to clients admitted to the dually certified OMH/OASAS crisis facility. The SCSC is a 24/7/365 program for individuals in Mental Health/Substance Use Disorder (MH/SUD) crisis situations. The SCSC will provide immediate, therapeutic support by certified peers and licensed behavioral health professionals. The RN Care Coordinator is part of a multidisciplinary team of staff and will support triage, screening, assessment, interventions, discharge planning and follow-up services for clients. The RN Care Coordinator is responsible for providing quality care to clients utilizing a person-centered approach for all individuals. DUTIES AND RESPONSIBILITIES Assist in the daily operations of the certified SCSC supporting individuals with Mental Health/Substance Use Disorder (MH/SUD) challenges Adheres to all state requirements and professional nursing standards as a licensed provider of medical care and treatment Triage, assess, and observe clients to determine appropriate level of care for individuals presenting at the SCSC for admission, using evidence-based screening tools and a trauma informed approach Monitor medical status of clients, including vital signs, basic medical care, examination and observation of signs/symptoms of medical conditions Provide therapeutic interventions to clients in crisis including psychoeducation, crisis counseling, and de-escalation Consult with on call Psychiatric Provider for medical/clinical advisement on psychiatric and/or substance use related concerns of clients, as applicable or as directed Maintain accurate records and complete documentation of client services Coordinate the appropriate care and services for clients, including developing and implementing Safety Plan and Individual Service Plan outlining care, treatment, and recommended services based on individual client needs at admission and upon discharge Provide information and referrals to treatment facilities or services for SUD Provide information and referrals to clinical/therapeutic services for mental health challenges Provide information and referrals for medical insurance and medical/primary care services Provide information and referrals to services related to social determinant of health needs (housing, financial, food security, transportation, childcare, utilities, safety, etc.) Collaborate with community partners to facilitate appropriate services upon discharge Communicate relevant client information with family members, caregivers, or designated individuals, as appropriate and if authorized Document information related to referrals, coordination of services, and follow-up contacts and supports provided to client and community providers Follow OMH and OASAS regulatory requirements and MHAST policies and procedures Help maintain overall cleanliness and safety of the facility; ensure a welcoming and supportive environment for clients Treat all clients with dignity and respect Demonstrate teamwork among co-workers and contribute to a multi-disciplinary team approach Attend agency staff meetings and trainings Cooperate with audits, inspections, investigations Flexibility to potentially cover shifts, in the event of call-ins or scheduling issues RN/Medical Care and Monitoring (30%) Coordination of Client Services (50%) Other (20%) WORKING CONDITIONS Crisis center facility, office environment Some exposure to cleaners, chemicals is expected Physical requirements include walking, bending, standing, climbing stairs, and lifting (light/medium-up to 50 pounds) May need to work flexible schedule, depending on staffing needs This job description is a summary of the typical job functions; it is not a comprehensive list of all possible responsibilities and duties. Requirements Associate’s degree or higher in nursing, with a valid RN License in NYS Two or more years of experience working in MH/SUD field Two or more years of experience working with and/or treating children, youth, and adults Obtain the NYS Peer Specialist Certification within orientation period Valid driver’s license and acceptable driving record to drive agency vehicles Successfully complete all required background and reference checks Complete all state/agency required trainings during orientation period, and annually, as required. Maintain strict confidentiality and professional boundaries Support the mission and vision of MHAST KNOWLEDGE/SKILLS/ABILITIES Knowledge of MH/SUD issues facing individuals in the community Knowledge and understanding of the characteristics, diagnosis, and treatment options of a mental illness and substance use disorders Ability to handle crisis situations, traumatic events, or sensitive circumstances, support and advocate for individuals in crisis Knowledge of community resources, services, and systems to assist individuals with referrals and links to appropriate care based on individual needs Understanding and sensitivity to support a diverse client population (race, religion, gender identity, sexual orientation, ethnicity, etc.) Trauma Informed Care Training Knowledge of EMR/EHR systems Knowledge of Medicaid/MCO billing Knowledge of OMH and OASAS Regulations Knowledge of Microsoft Office Suite (Excel, Word, Teams, etc.) Excellent organizational skills, detail-oriented Excellent communication skills (verbal, written) Benefits Sign-On Bonus Medical (Employer covers 90% of premium and 100% of deductible) Vision Short Term and Long Term disability Optional FSA, Aflac Paid Time Off Employee Assistance Program Life Insurance 401k with Matching Flexible Schedule #J-18808-Ljbffr
