Key Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions in a home health setting
• Complete evaluations to ensure quality standards and address assessment areas requiring attention
• Establish individualized treatment plans using appropriate clinical techniques, tools, and protocols
• Develop individualized care and discharge plans and communicate them to appropriate parties
• Complete comprehensive and accurate clinical documentation within established deadlines
• Supervise assigned therapy assistants and serve as co-leader in an interdisciplinary approach to patient care

Required Qualifications

• Active Hawaii state Physical Therapist license
• Minimum of 2 years of Physical Therapist experience
• Minimum of 1 year of home health Physical Therapy experience
• Bachelor's degree in Physical Therapy or related field
• Experience with Epic EMR
• Current CPR and BLS (AHA) certification

Preferred Experience / Skills

• Master's degree in Physical Therapy or related field
• Prior travel therapy assignment experience
