Physical Therapist (PT) Job DescriptionI. Purpose and Overall DutiesThe Physical Therapist (PT) evaluates, plans, and implements individualized physical therapy programs for patients in a long-term care or related setting. The role focuses on improving mobility, strength, balance, and functional independence while reducing pain and preventing further injury.II. Essential Job FunctionsA. Patient Evaluation and Treatment PlanningEvaluate patients’ physical condition, mobility, strength, balance, and functional abilities using standardized assessments and clinical judgmentDevelop individualized treatment plans based on patient needs, physician orders, and regulatory requirementsEstablish measurable goals and modify treatment plans as necessary based on patient progressB. Patient Care / InterventionImplement therapeutic interventions to improve mobility, strength, balance, coordination, and enduranceProvide gait training, transfer training, and therapeutic exercise programsUtilize manual therapy techniques and therapeutic modalities as appropriateEducate patients and caregivers on exercises, safety precautions, and home programsRecommend and train patients in the use of assistive devices (e.g., walkers, canes)C. Documentation / Compliance / OutcomesComplete all required clinical documentation accurately and timely in accordance with Medicare, regulatory, and company guidelinesMaintain appropriate records of patient evaluations, progress notes, and discharge summariesEnsure compliance with all federal, state, and payer regulationsMonitor patient outcomes and adjust interventions to support optimal resultsD. Collaboration / Supervision / LeadershipCollaborate with interdisciplinary team members to coordinate patient careSupervise and direct Physical Therapist Assistants (PTAs) and support staff as applicableCommunicate effectively with physicians, nursing staff, and families regarding patient progressParticipate in care planning meetings and team conferencesE. Essential Physical DemandsAbility to assist patients with mobility, transfers, and ambulation using appropriate techniques and assistive devicesAbility to stand, walk, and move throughout the workdayFrequent physical interaction with patients, including supporting or guiding movement during therapyOccasional bending, reaching, or positioning of patients during treatmentAbility to use proper body mechanics in accordance with safety standardsVI. Work EnvironmentLong-term care or clinical settingAbility to travel to various locations where patients are located, including long-term care, short-term care, and private homes. This work will involve ability to enter and access buildings, work with patient in their existing space. Varying environmental conditions may be encountered while traveling.Indoor environment with occasional travel between patient areas or facilities as neededVII.
