Develop Therapy Plans: Formulate personalized, evidence-based treatment and therapy plans in collaboration with the multidisciplinary medical team. Provide Crisis Intervention: Deliver immediate crisis intervention and stabilization for patients experiencing acute distress, including rigorous risk assessments for suicidal ideation. Deliver Health Coaching: Offer tailored, one‑on‑one behavioral health coaching to support patients in developing coping strategies and achieving long‑term wellness goals. What minimum qualifications you’ll need Licensure / Certification / Registration: BLS Provider preferred. American Heart Association or American Red Cross accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Counselor preferred. Social Worker preferred. Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
