Family & Aftercare Advocate

HOPE IS ALIVE MINISTRIES INC. seeks a Family and Aftercare Coordinator for The Healing Center to coordinate family involvement during residents’ 45-day stays and lead the development of aftercare plans. The role focuses on relationships, communication, coordination, and ongoing support without clinical treatment.

The coordinator will facilitate family education about Hope Is Alive resources, coordinate housing and community connections, and manage alumni follow-up after discharge.
