The clinic also provides a variety of specialty services, including audiology, physical therapy, and occupational therapy. The clinic is located on the campus of the Martinez VA Medical Center, which also offers inpatient care and rehabilitation services. Facility Address: 150 Muir Rd, Martinez, CA 94553ResponsibilitiesThe Physical Therapist shall be responsible for planning, reviewing, and implementing treatment programs based upon initial and interim evaluations.Communicates and collaborates with patients, family members, caregivers, and interdisciplinary professionals.Adapts assessment tools and treatments to the complexity of the diagnosis or disabilities and demonstrates the clinical reasoning necessary to identify the need for further in-depth specific assessment of function and utilization of non-standard methods and techniques.Documents plan of care and care given in patient's medical record.Monitors patient's response to interventions, modifies treatment plan, and reevaluates as indicated to meet short-term and long-term goals.Expected to interpret findings from examination and evaluation, select appropriate tests and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations.Measures patients for assistive devices and special equipment.Fits devices for patient needs and instructs patient/care providers in the use of these devices.The Physical Therapist develops home programs for patients as needed prior to discharge and instructs patients, family, or caregivers in implementation of home program.Provides consultation, as needed, to peers and other staff responsible for the care of the assigned patient.Provides clinical oversite and collaboration with physical therapy assistants.Provides patient care which is appropriate to cognitive, emotional, and chronological maturation needs of young, middle, and older adults' patients.Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.The Physical Therapist maintains equipment and clinics in a safe and orderly manner in accordance with safety and infection control policies, Joint Commission, and other relevant policies.RequirementsExperience: Shall have a minimum of 1 year of inpatient rehabilitation or therapist experience.Certification: Must hold current certifications in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) or equivalent.License/Registration: Shall possess a current, valid, unrestricted physician license in the state, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.KurzSolutions is committed to improving health outcomes by providing well-managed companies with the top healthcare talent in the market.
