Responsibilities and job duties: As an RN perform the initial homecare visit and re-evaluate the patient’s needs and progress on a regular basis. Initiate the plan of care under doctor’s orders. Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient. Perform medication reconciliation. RN observe and assess patients, and document patient assessments. Monitor reactions and patient progress to treatment. Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans. Educate patients and caregiver on techniques for in home health care. Communicate and coordinate with other members of the healthcare team. RN to supervise LPNs and HHA’s and provide oversight of the care plan. Provide onsite Supervision of LPN & HHA. Notify the physician and other personnel (Clinical Manager, Therapist, and Case Manager) of change in the patient’s condition. Perform skill outline in the agency’s approved policy and procedure manual. Discharge the patient from skilled nursing services when the discharge criteria have been met. Case conference clinicians providing care to ensure coordination of care. Update clinical records according to policy and procedures. Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually. Adhere to Medicaid and Medicare regulations. Skills Proven experience as a registered nurse Excellent knowledge of nursing care methods and procedures Excellent knowledge of home healthcare In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times A team player with excellent communication and interpersonal skills Responsible and compassionate Outstanding organizational and multi-tasking skills Patient with excellent problem-solving skills Qualifications: Nursing: 2 years (Required) Valid and Current Missouri RN license Scheduling and Shift Details Offering both full and part time positions. Shift lengths anywhere from 4hrs-12hrs. Start times are flexible. Part-time 6-31 hours each week. Full-time 32-40 hours each week. #J-18808-Ljbffr
