Registered Nurse- Per Diem

Responsibilities Assess and identify patient/family needs using the nursing process. Coordinate the Plan of Care with the Interdisciplinary Group (IDG) and provide palliative and supportive care. Conduct clinical admissions: explain services, identify objections, educate, obtain consents, document, and initiate the Plan of Care. Act as the Company representative at assigned facilities, facilitate referrals, and work closely with hospitals, physicians, and families. Communicate with Admissions to ensure timely follow‑up, inform Admissions when assignments are complete, and prepare for the next assignment. Provide clinical communication using SBAR and discuss potential needs with after‑hours staff. Obtain consents, arrange safe discharge, and obtain medical record information for referrals. Develop strong relationships with case managers, physicians, and other facility staff. Assist Professional Relations Representatives (PRRs) in providing education on all available services. Provide care for GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge. Attend patient deaths, prepare bodies for transport, oversee disposal of medications, and provide bereavement interventions. Supervise patient care given by other nursing team members such as LPNs, Hospice Aides, and contract nurses. Respond to emergencies, reporting early if required to ensure continuity of services. Perform other duties as assigned. Qualifications Current RN license in the state where the employee will be working. Active BLS certification for healthcare professionals. Minimum one year of nursing experience; hospice or hospital experience preferred. Experience working with an EMR/EHR system. Valid driver’s license and automobile insurance per Company policy, plus reliable transportation. Ability to use equipment with visual and auditory mechanisms. Proficient in effective communication in English, both verbal and written. Flexible schedule to meet the needs of Hospice House patients. Capacity to manage the emotional stress of caring for terminally ill patients and their families. Physical ability to access patient rooms, hospital areas, and homes—including navigating stairs, narrow spaces, heat, and lack of air conditioning. Capability to perform essential functions such as lifting, bending, pushing, pulling, and kneeling, with or without reasonable accommodation. For employees working at PACE, certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs. Cross‑training readiness for hospice house or field positions of equal licensure/certification and scope when census calls for flexible staffing.
