About the Role

As an LPN, you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and conducting routine assessments. You will also collaborate with our interdisciplinary team to develop and implement individualized care plans, ensuring our residents receive the highest level of care possible. Your strong communication skills will be essential in maintaining open lines of communication with patients, families, and healthcare professionals.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and conducting routine assessments
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct routine rounds to monitor patient progress and identify areas for improvement
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

Ideal Candidate

• Must be a Licensed Practical/Vocational Nurse (LPN/LVN) with a current license to practice in the state of Ohio

• Possess strong communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced, dynamic environment

• Strong attention to detail and ability to maintain accurate records
