Physical Therapist – Home Health- Home Sweet Home
Location: West Valley Region, Peoria, AZ and surrounding areas
Compensation: Per Visit | Earn up to $103,500 annually based on productivity
Sign-On Bonus: $5,000
Schedule: Per Diem, Part - Time, and Full time

You will provide one-on-one care in patients' homes, helping them regain strength, mobility, and confidence in their daily lives.
Key Responsibilities- Conduct in-home evaluations, start of care, and follow-up visits
- Develop and implement individualized treatment plans
- Improve strength, balance, mobility, and functional independence
- Educate patients and caregivers on safety and equipment use
- Document visits using WellSky Kinnser EMR
- Collaborate with physicians, RNs, and the care team
- Active Arizona Physical Therapist license
- CPR certification (AHA)
- Minimum 1 year of clinical experience (home health preferred)
- Strong clinical judgment and time management
- Reliable transportation, valid driver's license, and insurance
- Strong communication and a patient-first approach
- Flexible, independent schedule
