Pharmacist (Clinical Staff Pharmacist)

The Pharmacist is responsible for providing comprehensive pharmacy services, ensuring safe and effective medication therapy management, and supporting quality patient care initiatives. The pharmacist works closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes in an ambulatory and outpatient setting.

Key responsibilities include:

• Dispense medications accurately and efficiently in accordance with federal, state, and IHS regulations.
• Review prescriptions for appropriateness, drug interactions, allergies, and therapeutic duplications.
• Provide medication counseling and education to patients and family members.
• Conduct medication therapy management (MTM) and clinical interventions.
• Collaborate with providers to optimize pharmaceutical care and treatment plans.
• Monitor patient medication profiles and make evidence-based recommendations.
• Participate in chronic disease management programs, including diabetes, hypertension, and anticoagulation monitoring.
• Maintain accurate pharmacy records and documentation within the electronic health record (EHR).
• Ensure compliance with DEA, Joint Commission, IHS, and pharmacy practice standards.
• Assist with inventory management, controlled substance accountability, and medication procurement.
• Participate in quality improvement initiatives and pharmacy staff education.

Qualifications:

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy.
• Active, unrestricted pharmacist license in any U.S. state or territory.
• Ability to obtain and maintain federal credentialing requirements.
• Strong clinical judgment and attention to detail.
• Excellent communication and patient education skills.

• Hospital, ambulatory care, or tribal health experience.
• Residency training (PGY1) preferred but not required.
• Experience with electronic medical records and interdisciplinary healthcare teams.
• Board certification (BCPS, BCACP, etc.) a plus.
