MDS/Care Plan Coordinator Job Summary: As a MDS/Care Plan Coordinator at Oak Park Oasis, you will play a vital role in ensuring seamless patient care by creating and implementing comprehensive care plans for our residents. With a focus on reducing re-hospitalization rates and promoting independence, you will collaborate with our multidisciplinary team to develop individualized plans that meet each patient's unique needs. Responsibilities: Coordinate with healthcare professionals to develop and implement Individualized Care Plans (ICPs) for each patient, ensuring compliance with Medicare and Medicaid regulations Conduct comprehensive assessments and evaluations to identify patient needs and develop strategies for meeting those needs Collaborate with multidisciplinary teams to develop and update care plans, ensuring continuity of care and effective communication with patients, families, and healthcare providers Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes Coordinate with outside services, such as home health care and durable medical equipment, to facilitate a smooth transition back to the patient's home Maintain accurate and detailed records of patient assessments, care plans, and progress Participate in quality improvement initiatives and maintain confidentiality and adhere to HIPAA guidelines Requirements: 2 years of experience in a related field, with a strong understanding of MDS/Care Plan coordination principles and practices Licensed or Registered Nurse in the State of Illinois (unencumbered) High school diploma or equivalent required; associate's or bachelor's degree in a related field (e.g., healthcare administration, rehabilitation science, or nursing) preferred Strong analytical and communication skills, with the ability to work effectively with patients, families, and healthcare professionals Ability to adapt to changing priorities and deadlines in a fast-paced environment Basic computer skills and proficiency in electronic medical records (EMRs) What We Offer: A dynamic and supportive work environment with a focus on patient-centered care Opportunities for professional growth and development in a rapidly evolving healthcare industry
