Client Care Coordinator – Home Care

Build trusted relationships. Guide families through important decisions. Help older adults remain safely at home.

Comfort Keepers of Anoka, White Bear Lake, and Chisago Lakes is looking for a relationship-driven Client Care Coordinator who can quickly connect with seniors and their families, understand their needs, and confidently guide them toward the right care.

This is not simply an administrative care-coordination position. You will meet families during important—and sometimes difficult—moments. Your ability to listen, read the situation, build trust, and recommend practical solutions will directly affect each client’s safety, independence, and quality of life.

Comfort Keepers is a leading provider of in-home senior care, private duty nursing, and adult care in the north metro Twin Cities. We help seniors, veterans, and adults remain safe, comfortable, and independent in their own homes while giving their families greater confidence and peace of mind.

We serve families throughout the Anoka, White Bear Lake, and Chisago Lakes areas with warmth, compassion, professionalism, and a commitment to doing what is right for each client. This position is based in White Bear Lake and covers that geographic area.

About the Role

As a Client Care Coordinator, you will be one of the primary faces of Comfort Keepers for clients, families, and community partners. You will assess client needs, develop personalized Plans of Care, maintain strong relationships with existing clients, and help families adjust services as their circumstances change.

You will also represent Comfort Keepers in hospitals, transitional care facilities, senior communities, and professional networking settings. By providing excellent service and building genuine relationships, you will help more families learn how Comfort Keepers can support them.

Above all, you will advocate for the best possible outcome for the client.

What You’ll Do

Build trusted relationships with clients and families

• Meet with prospective clients and families in their homes, hospitals, transitional care facilities, or virtually.

• Listen carefully to their concerns, priorities, family dynamics, and care goals.

• Explain home care options clearly and recommend appropriate services without using a high-pressure approach.

• Help families feel heard, supported, and confident in their decisions.

• Maintain regular contact with clients and families throughout the care relationship.

Coordinate excellent client care

• Conduct assessments and develop personalized Plans of Care.

• Communicate client needs clearly to caregivers, nurses, operations staff, and other members of the care team.

• Visit clients when they are hospitalized or receiving transitional care to understand changing needs and assist with a safe return home.

• Monitor client satisfaction, safety, and changing care needs.

• Recommend adjustments when additional support would improve the client’s outcome.

• Address concerns promptly and work collaboratively to find practical solutions.

• Maintain timely, accurate documentation in our client-management and CRM systems.

Develop community and referral relationships

• Build productive relationships with social workers, discharge planners, rehabilitation teams, senior communities, physicians’ offices, and other referral partners.

• Represent Comfort Keepers at networking meetings, health fairs, community events, and educational programs.

• Follow up consistently with professional contacts and prospective clients.

• Identify opportunities to educate the community about senior care and help families access support earlier.

• Track referral activity and relationship-development efforts in our CRM.

Qualifications

Required

• Three or more years of relevant experience in senior care, home care, healthcare, client services, care coordination, community relations, consultative sales, or a similar relationship-based role. Some medical knowledge is necessary.

• Demonstrated ability to build trust with clients, families, and professional partners.

• Strong verbal and written communication skills.

• A natural curiosity about things. 

• Confidence discussing sensitive family, health, safety, and financial concerns.

• Sound judgment and respect for client privacy and professional boundaries.

• Strong organization, documentation, and follow-through.

• Working knowledge of Microsoft Office and the ability to learn CRM and care-management platforms.

• Valid driver’s license, reliable transportation, and willingness to travel throughout our north metro service area.

Preferred

• Previous home care, senior living, healthcare, social services, or care-management experience.

• Experience conducting client assessments or developing Plans of Care.

• Experience working with hospitals, rehabilitation centers, social workers, or discharge planners.

• Experience leading presentations, educational programs, or community events.

• Bachelor’s degree in healthcare administration, social work, business, marketing, human services, or a related field.

Schedule and Work Location

This is a full-time, primarily in-person position involving office work, client home visits, hospital and transitional care visits, and community events throughout the Anoka, White Bear Lake, and Chisago Lakes service areas.

Typical schedule: This is a full-time, exempt position. Regular office hours are typically 8:00 a.m. to 4:30 p.m., Monday through Friday. Occasional attendance at care conferences, professional meetings, trade shows, community events, and other assigned activities may be required outside regular office hours. We strive to do our jobs during regular hours Monday through Friday
