Licensed Practical Nurse (LPN) - Home Health

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide exceptional care to patients in their homes. In this role, you will collaborate with a multidisciplinary team to ensure the highest quality of patient-centered care.

Home Health LPN Responsibilities:

• Maintain professionalism and strong relationships with patients, visitors, and colleagues.
• Conduct patient revisits under the Team Clinical Manager's supervision.
• Collaborate with RNs and other team members to ensure coordinated care.
• Communicate effectively to promote patient-centered care.
• Demonstrate strong writing, computer, and organizational skills.
• Identify and address safety issues for staff and patients.
• Engage empathetically with a diverse patient population.
• Ensure accurate data collection and record-keeping.
• Present a neat and professional appearance.
• Adhere to attendance policies.

Home Health LPN Requirements:

• Minimum of one year of nursing experience; home care experience preferred.
• Must possess a valid driver's license and automobile insurance as required by law. Candidates must be at least 21 years old to meet fleet vehicle insurance policy requirements.
• LPN License and BLS Certification are required.
• Possession of professional liability insurance is encouraged.
