POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Responsible for discharge planning from inpatient units

MINIMUM REQUIRED QUALIFICATIONS –

• NY RN license
• 5 years acute care case management experience preferred
• Hospital discharge planning experience required
• Patient/family communication regarding post-acute arrangements experience required
• Knowledge of hospice and end of life care principles preferred

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7.5hr days, M-F 

SYSTEMS – Epic

START DATE – ASAP
