Family Medicine Provider Float

United Health Services (UHS) is seeking a board certified Nurse Practitioner (NP) or Physician Assistant (PA) for a full time Monday-Friday Family Medicine Provider Float position. This position will have a home base at our UHS Primary Care Upper Front Street site and travel would be limited to assigned practices within a 30-mile radius of the home base practice- Upper Front St, Greene, and Clinton Street Medical Centers.

UHS Primary Care Practices are Nationally Recognized by NCQA and a Certified Patient Centered Medical Home. Our Primary Care practices provide health promotion, disease prevention, health maintenance, patient education, diagnosis and treatment of acute and chronic illnesses for patients of all ages.
