The position requires an in-person Case Manager serving the Yakima or Inland Northwest geographic areas.FTE: 1.0, Shift: Day, Schedule: Monday - FridayPosition SummaryThe Hospital Case Manager RN is responsible for high quality cross continuum care coordination. This individual works with physicians/APP, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health System's commitment to patient centered care and community engagement.ResponsibilitiesEnsure that the patient and family experiences medically appropriate transitions of care, and an integral and essential component of the care delivery processAdvocate for patients and families, including transition to lower level of care when inpatient care is no longer neededPartner with and supports physicians/APP in patient care activities and care transitionsUtilize critical thinking in all interactionsBe accountable for transitions of care and care across the continuumRequirementsBachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreementAdvanced degree in Nursing (MSN, PhD, DNP, ARNP) preferredCurrent Registered Nurse license in Washington State or Multistate License endorsement (MLS)Four (4) years recent hospital case management experienceFive (5) years of recent clinical hospital/ambulatory experienceHealthcare financial and reimbursement knowledge preferredLeadership and emergency services experience a plusConsideration may be given to internal candidates not meeting the minimum qualificationsA Better Way to Work in Health CareFlexible schedule: Make a positive difference in your community and have time and energy to pursue your passions outside of work.Team-based care model: Be part of a team that is dedicated to excellence - not only for our customers, but for each other. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $101,407.00 - $177,821.28 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
