PRN - Physical Therapist

The Rehab Center is looking for a PRN Physical Therapist to join our interdisciplinary team, which provides treatment to individuals with chronic pain, concussion/brain injury, and other work related conditions. Treatments are individualized to help our patients gain improved function and independence, with the goal of returning to productive, fulfilling lives. We treat our patients through acute, sub-acute, OP care, work hardending, work conditioning, or our 20 day interdiciplinary program, depending on the need of the patient. Duties and Responsibilities Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes. Conducts therapy practice within defined standards of care. Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures. Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature. Required Skills and Abilities Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services. The ability to collaborate with the interdisciplinary team members, case managers, and other treatment providers. Excellent interpersonal and communication skills. Physical Requirements Duties require extensive standing and walking. Requires lifting, positioning, pushing, and/or transferring patients. Duties require frequent reaching, stooping, bending, kneeling, and crouching.
