Candidate Type: Traveler
Requirements: one full year of recent adult home health or hospice experience depending on specialty. CPR and BLS certifications. Must be willing to travel up to 75 miles from the home clinic. Active license in hand. Proficiency in Homecare Homebase (HCHB) within the past six months; clinicians with prior HCHB experience outside this window must complete retraining. RNs, PTs, and OTs must be proficient in OASIS.
Responsibilities:

• Provide comprehensive hospice patient care through home visits.
• Attend periodic office meetings and documentation using HCHB.
• Travel up to 75 miles from the home clinic, covering Wood County.
• Manage an average caseload of 5 to 8 patients daily.
• Ensure compliance with agency protocols and documentation standards.
