Job Summary:

Responsibilities:

• 
  Administer medication and treatments per physician orders and facility protocols.
  
  
• 
  Monitor, record, and report patient vital signs and health status.
  
  
• 
  Assist with daily living activities, including bathing, dressing, and mobility.
  
  
• 
  Ensure infection control procedures are followed consistently.
  
  
• 
  Document patient care accurately and promptly.
  
  
• 
  Collaborate with healthcare team members to develop and update care plans.
  
  

Qualifications:

• 
  Valid LVN or LPN license in the state.
  
  
• 
  Previous experience in LTC or SNF settings preferred.
  
  
• 
  Ability to perform clinical tasks with attention to detail and compassion.
  
  
• 
  Strong communication and interpersonal skills.
  
  
• 
  Complete required immunizations, including COVID-19 vaccination, or have documented exemptions.
  
  
• 
  Ability to bring a valid photo ID and wear a photo badge at all times.
