Wound Care Nurse
Position Summary

The Wound Care Nurse is responsible for assessing, planning, implementing, and evaluating wound care for residents in accordance with physician orders, facility policies, and state and federal regulations. This position collaborates with the interdisciplinary team to promote optimal wound healing, prevent skin breakdown, and improve residents' overall quality of life. The Wound Care Nurse also serves as a clinical resource and educator for nursing staff regarding wound prevention and treatment.

Essential Duties and Responsibilities

• Perform comprehensive skin and wound assessments upon admission, readmission, and as clinically indicated.

• Develop, implement, and update individualized wound care treatment plans in collaboration with physicians and the interdisciplinary team.

• Provide direct wound care treatments, including dressing changes, negative pressure wound therapy (wound vac), debridement assistance (within scope of practice), and other physician-ordered treatments.

• Monitor and document wound healing progress, measurements, drainage, staging, and treatment effectiveness.

• Ensure accurate and timely documentation in the electronic medical record (EMR).

• Coordinate with physicians, nurse practitioners, and specialists regarding changes in wound status and treatment recommendations.

• Participate in weekly wound rounds and interdisciplinary care plan meetings.

• Identify residents at risk for pressure injuries and implement preventative interventions.

• Educate residents, families, and staff on wound prevention, treatment, pressure injury prevention, and skin integrity management.

• Ensure compliance with infection prevention and control practices related to wound care.

• Monitor and maintain wound care supplies and equipment.

• Assist with quality assurance initiatives related to skin integrity and wound management.

• Support survey readiness by ensuring documentation and wound care practices meet CMS, AHCA, and facility standards.

• Participate in staff education and competency training related to wound care protocols.

• Collaborate with the Director of Nursing and Unit Managers to improve wound care outcomes.

• Perform other duties as assigned.

Qualifications

• Current Florida Registered Nurse (RN) license required. Licensed Practical Nurse (LPN) may be considered based on facility needs and applicable state regulations.

• Current CPR/BLS certification.

• Minimum of one (1) year of nursing experience in a skilled nursing facility, long-term care, rehabilitation, or acute care preferred.

• Previous wound care experience strongly preferred.

• Wound Care Certification (WCC, WOCN, or equivalent) preferred or willingness to obtain within a designated timeframe.

• Knowledge of pressure injury prevention, wound healing principles, infection control, and skin integrity management.

• Strong assessment, organizational, communication, and documentation skills.

• Proficiency with electronic medical records.

Physical Requirements

• Ability to stand, walk, bend, reach, and lift up to 50 pounds with or without reasonable accommodation.

• Ability to assist with resident positioning and transfers as needed.

• Manual dexterity sufficient to perform wound treatments and dressing changes.

• Ability to wear appropriate personal protective equipment (PPE).

Work Environment

• Skilled Nursing Facility and Rehabilitation Center.

• Frequent exposure to healthcare environments requiring infection control precautions.

• May require weekends, holidays, and on-call responsibilities based on facility needs.

• Comprehensive understanding of wound assessment, staging, and treatment modalities.

• Knowledge of CMS regulations, Florida AHCA requirements, and long-term care standards.

• Strong clinical judgment and critical thinking skills.

• Ability to prioritize multiple tasks while maintaining high-quality resident care.

• Commitment to resident-centered care and positive clinical outcomes.

Licensure & Certifications

• Active Florida RN license required (LPN considered if permitted by facility policy and state regulations).

• Current BLS/CPR certification.

• Wound Care Certification (WCC, WOCN, CWCN, or equivalent) preferred.
