Growing together.As a Hospice Admissions RN, you will guide patients and families through the admission process by assessing physical, psychosocial, and spiritual needs and helping establish an individualized plan of care. You will collaborate with physicians, facilities, and the interdisciplinary team to coordinate services, provide skilled interventions and education to clients and family.Primary Responsibilities:Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needsCoordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary teamProvides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physicianFunctions as a liaison between disciplines and facilities during the admission processDevelops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriateCommunicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as requiredPerforms on-call responsibilities and on-call services to patients/families as assignedPerforms other duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current RN license, specific to the state(s) you are assigned to work2+ years of clinical RN experience in acute healthcare setting(s)Current CPR certificationValid driver's license, reliable transportation, and liability insurancePreferred Qualifications:Solid communication and interpersonal skillsDemonstrated ability to work independently and manage multiple prioritiesComfortable with daily travel between facilitiesProficient in electronic medical record (EMR) systemsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $79,600 to $119,400 annually based on full-time employment.
